Your treatment plan is uniquely tailored to you—obtained from your initial evaluation. If medications are recommended, your provider will select the best option based on your goals and personal lifestyle.
the “signature follow-up” visit
Your provider will evaluate and monitor your progress, track improvement of symptoms, establish new goals, discuss any side effects, and answer any questions you may have about your treatment plan.
the “sweet spot” visit
When your functional status is back to baseline and you’re feeling balanced and well. Designed exclusively once you are on a stable regimen. Ask your provider if this is the right visit for you.
Individuals who choose to pay out-of-pocket or who have not met their yearly deductibles, whether their insurance is in-network or out-of-network, will be responsible for full payment of the visit, due at the time of service. Copays will also be due at the time of service. If your appointment lasts longer than your scheduled time, you may be charged for the extra time. Additionally, it is the responsibility of the patient to know what his/her/their mental health benefits are and if his/her/their insurance carrier will reimburse him/her/them to see an "out-of-network" or "non" provider and for how much.
A $100.00 fee will be charged to your credit card for any missed office visits, whether they be in-person or virtual, if appointments are cancelled within 24 hours of your scheduled visit or if you are greater than 10 minutes late to your scheduled appointment.
If payment is not collected in full within 120 days of any unpaid balances, elevate medical, llc has the right to send your account to a collection agency, cancel any future appointments and refer you to another provider.
Fees are subject to change.
